Whistleblowing Form
This form may be used by Associates or third parties (such as suppliers, guests, and contractors) who wish to make a report under WEIL Hotel’s Whistleblowing Protection Policy.
This form is designed to allow you to report any suspected or known misconducts, illegal acts, unsafe work practices, violation of law or regulations or any breaches of company policies involving persons employed by WEIL Hotel.
In order for us to properly investigate your report, please provide complete and accurate details of the matters complained of and, where possible, evidence to support the complaint. Where witnesses are available, please provide the names of such witnesses.
In accordance with the Policy, we are unable to conduct fair and thorough investigations if your complaint is anonymous. Therefore, please provide your name and contact details so that we can contact you to follow up on the matter raised in this form. In accordance with the Policy, the information provided in the form, including your name and contact details, will be protected and kept confidential and will not be disclosed to third parties unless required by law.
Please be advised that knowingly providing false or misleading information is a serious offence and will not be tolerated. Associates found to have done so may be subject to disciplinary action, up to and including suspension or termination of employment.
This form is not intended for complaints related to personal circumstances, workplace grievances, individual disputes, or concerns about financial or business decisions made by WEIL Hotel. If you are an Associate of WEIL Hotel, please address such matters with the Human Resources Department or through the WEIL Grievance Procedure.
Important Note:
All reports will be treated with strict confidentiality. We prohibit retaliation against individuals who report concerns in good faith. Providing false information may result in legal consequences.
